Program Overview
Child and Family Connections (CFC), through the Lake County Health Department, serves families with children under the age of 3 with suspected developmental delays. CFC provides a system point of entry into the Illinois Early Intervention System. CFC provides service coordination of early intervention services including evaluations, connecting eligible families to therapies, community resources and transition.
During this session, attendees will learn what Early Intervention is, hear the parent perspective, and gain a practical understanding of how families and professionals can make referrals and access Early Intervention services. Learn about services and resources available for children ages 0-3 experiencing a physical or mental condition that creates developmental delays.
Meet our Presenter
Stephanie Jimenez, with the Lake County Health Department, is the Parent Liaison at Child and Family Connections #2, where she supports families in navigating Illinois’ Early Intervention program. Early Intervention is a birth to three program that serves infants and toddlers who have developmental delays or disabilities, as well as those at risk for delays.
